To assist Turanganui A Kiwa whanau in the early identification and intervention of heart disease, lung disease and diabetes
Disease State Management Programme provides individuals and whanau with education and support on:
- Heart Disease
- Lung Disease
- Diabetes
To self manage their disease and wellness by an advanced Registered Nurse Practitioner.
There are regular clinics providing FREE health checks and education in the following rural communities:
- Patutahi
- Manutuke
- Muriwai
- Mangatu
- Matawai
- Te Karaka
