Home & Community Support Service

Service Delivery Goal

Helping whanau to maintain well being and independence in the safety of their own home, also during transition from home into a supported independent setting

Helping Whanau (Family) to be safe and independent in their own home.

Access To Service:

This service is accessed through a referral from:

  • District Nurses
  • ACC
  • Life Unlimited
  • Tairawhiti District Health
  • Rural District Nurses

Criteria of Eligibility:

Individuals must have diagnosis of:

  • Personal Health Illness / Disease
  • Terminal Illness
  • Sensory/Physical / Intellectual Disabilities
  • Accident Related Injuries

If you are unsure whether you meet the above criteria please contact Home & Community Support Service and a referral will be made to the appropriate service.

Services Provided:

All services are provided by trained caregivers

  • Home Help Assistance
  • Meal Preparation
  • Personal Care
  • Community Participation
  • Supported Independence Living

These services are only provided once the assessment is complete and confirmation of allocated hours are received from the referring agency.

All cleaning materials and equipment are supplied by client.
Hours of Service
Monday-Friday 8.00am - 5.00pm:
On Call hours Monday-Friday 7.00am - 8.00am and 5.00pm - 7.00pm, Saturday-Sunday 7.00am-7.00pm.
