E Tipu E Rea Clinical Coordinator

We have a vacancy in our E Tipu E Rea (ETER) service for a part time (20hr per week) Hub Clinical Coordinator.

The E Tipu E Rea (ETER) Clinical Coordinator has been established for the provision of clinical support for māmā and or her tamariki identified within the ETER program. 

This role will provide comprehensive, appropriate and timely clinical intervention by working with the hub administrator to support the three community based ETER service providers – Turanga Health, Ngati Porou Hauora and Hauiti Hauora. 

The role entails:

* receiving, capturing, processing and allocating referrals
* connecting with whānau when required
* analysing data and trends
* capturing whānau voice to evaluate the program to guide changes and improve service delivery
* work with other community service providers to ensure good connection for whānau within the scope of ETER.

The person in the registered nurse role will provide  nursing services including but not limited to:

* assessments
* whānau plans
* management and treatment of registered whānau
* reviews and evaluation
* referrals to other internal and external services.
